Community Living Transition Grants

Training funds are available for a sixth year through a grant from the Iowa Department of Human Services to the Iowa Training Consortium.

The Consortium, formed after the 1994 Conner Decree, is a statewide group that promotes training to build supportive communities for Iowans who have disabilities. The Office of Community Education at University Hospital School manages the grant for the Consortium.

The 2002-2003 grant will focus on supporting specific individuals to live in their communities, rather than in institutional settings.

$20,000 is available to fund one or two grants to community coalitions to assist in transitioning an individual from a large congregate facility to community-based living.
The number of grants to be funded will depend on the quality of the proposals and budgets submitted.

Proposals must include:

A description of the person who will be the focus of the project
A letter of agreement from the person who will be the focus of the project
A step-by-step description of the transition process
A detailed budget
A description of how the transition process will be documented
A plan to support the placement after the grant funds are used
Support letters from members of the community coalition that will help with the project.

Proposals will be judged on the following criteria:

The degree of participation and choice by the individual being transitioned
The quality and commitment of the community coalition
The quality and practicality of the transition plan
The quality of the plan to document the process
The quality of the plan to maintain long-term support for the placement.

Choice: consumers and their families have the ability to exercise informed choices about the amounts and types of services and supports received.

Community: the service system should support the rights and the ability of all consumers to live, learn, work, and recreate in natural communities of their choice.

Empowerment: the system should reinforce the fundamental rights, dignity, and ability of consumers to accept responsibility, make choices, and take risks.
